Liability Waiver WARNING Under Florida law, an equine activity sponsor or equine professional is not liable for an injury to, or the death of, a participant in equine activities resulting from the inherent risks of equine activities. FL ST § 773.01 – 773.05 I hereby acknowledge that I have read the above warning. I agree to this Waiver and Liability Release. I agree, without any limitation or condition, to hold Equinox Equestrian Center (also known as EEC,LLC) its owners, staff and employees, as well as any organizers, judges, officials or volunteers assisting at clinics, shows or lessons hosted by EEC, harmless from any and all liability, loss, claims or actions, causes of action, judgements or demands of any nature whatsoever. Further, I understand that all equine sports, including driving and this competition, and any proximity to equines, involve inherent risks that may include, but are not limited to, serious injury, and even death. By participating I do so voluntarily and expressly assume any and all risks of injury to me or my equines, or loss of animals or equipment. I agree to indemnify and hold harmless EEC, its owner, employees and staff, as well as organizers, coaches, clinicians, volunteers, competition judges and officials, volunteers and assistants from any and all claims for loss or injury to myself or my horse(s) that may occur during or in conjunction with this competition or event. Helmets: I understand that Florida law [F.S.A. §773.06] requires children younger than age 16 to wear properly fitted and securely fastened ASTM-standard equestrian protective headgear while riding equines at various locations described in the law. I also understand that while it is not required by law, the owners of EEC encourage all participants to wear a properly fitted and secured ASTM-standard/SEI certified protective headgear that is designed for use when riding, driving, or near equines. I agree to be bound by the rules of the facility and by the dictates of the competition officials. I acknowledge that they retain the right to require my animals and myself to leave the premises for any behavior deemed unsafe or inappropriate. All dogs and small children must be on a leash or otherwise contained. No harassment of animals, officials or other beings will be tolerated. By signing below, I agree to be bound by all the terms and conditions of this agreement.
General Photo Release: I hereby agree/do not agree that EEC may use or assign any images of my horse or myself, in any format, taken at this event, with or without including my name by the facility or by any services approved by Equinox Equestrian Center (EEC). EEC may use my photograph, at its discretion and consistent with its services, in any publication and/or on an Internet or in any other format. I understand that other persons will be free to copy and/or print and/or distribute my photographic image.
PERMISSION FOR MINOR TO SHOW I hereby consent to the entry of my child in this horse show and certify that I have read the foregoing representations and statements and that the same may be deemed a part thereof, and hereby accept responsibility thereunder for the participation of said minor.
